Clear Choice Insurance — Website Terms of Service

Effective date: September 1, 2026
Website operator: Clear Choice Insurance, LLC

1. About these terms

These Terms of Service apply to your use of the Clear Choice Insurance website and its online inquiry features. “Clear Choice,” “we,” “us,” and “our” refer to Clear Choice Insurance, LLC. Please read these terms before using the website. If you do not agree, do not use the website; you may contact our agency directly for assistance.

This website does not offer customer accounts or accept online payments. Separately operated insurer or service-provider websites may offer those features under their own terms.

These terms address website use. They do not replace an insurance policy, binder, endorsement, application, or separate agreement for agency services. Nothing here changes an insurer’s obligations under a policy or limits rights or duties that cannot lawfully be waived.

2. Our role as an independent agency

Clear Choice Insurance is an Arizona independent insurance agency that helps customers explore insurance options from multiple insurance companies. We are not the underwriting insurance company. Coverage is issued by the insurer identified in the applicable policy.

We do not represent every insurance company or compare every product available in the marketplace. Available insurers, products, and services depend on our appointments, licensing, your circumstances, and underwriting requirements. We offer insurance services only where appropriately licensed and authorized.

3. Website information and quotes

Website descriptions provide general information. They are not complete descriptions of coverage and do not guarantee eligibility, availability, a particular premium, or savings. Actual coverage is subject to the policy’s terms, conditions, limits, exclusions, and endorsements.

Any preliminary estimate is subject to verification, underwriting, and the applicable insurer’s approval. Premiums and availability may change before coverage becomes effective. An agent can help you review options, but the insurer’s issued documents determine the coverage provided.

4. Requests do not bind or change insurance

Submitting a website form, requesting a quote, sending an email, or leaving a voicemail does not, by itself, create, bind, renew, change, or cancel coverage. An automated acknowledgment confirms only receipt or processing of a message; it is not proof of insurance or confirmation that an instruction has been accepted.

Do not assume that coverage or a requested change is effective until you receive confirmation from an authorized representative or the insurer identifying the accepted coverage and effective date. If you need time-sensitive assistance, contact the agency or insurer directly and confirm that your request has been received and acted upon.

5. Claims, payments, and urgent matters

Follow your policy’s instructions for reporting claims, making payments, and meeting deadlines. Unless your insurer expressly designates this website as an authorized channel, submitting an agency inquiry does not constitute notice of a claim to the insurer or satisfy a payment or policy deadline.

For emergencies, call 911 or the appropriate emergency service. This website is not an emergency reporting service and may not be monitored continuously.

6. Information you provide

Provide accurate, current information and promptly tell us about material errors or changes relevant to your request. You must have authority to provide information about another person or business. Inaccurate or incomplete information may affect a quote, eligibility, or coverage as provided by applicable law and policy terms.

Do not send Social Security numbers, driver’s license images or numbers, payment-card details, medical records, passwords, or other sensitive documents through a general contact form or ordinary email. Ask us for an appropriate method if such information is needed.

Our Privacy Policy explains how we handle personal information. These terms do not substitute for any specific authorization required to obtain records, disclose insurance information, or send marketing communications.

7. Communications

When you ask us to contact you, we may respond using the contact information you provide about that request. An inquiry does not by itself authorize unrelated marketing, automated marketing calls, or marketing text messages. Where separate consent is required, it must be obtained separately.

Electronic messages can be delayed, filtered, or misdirected. Contact us directly if a request is urgent or you have not received an expected response. Do not rely on a website acknowledgment to meet a policy deadline.

8. Permitted use and intellectual property

You may use the website for lawful personal or business insurance inquiries. You may not impersonate another person, submit fraudulent information, attempt unauthorized access, introduce malicious code, interfere with website operation, or use the website to unlawfully collect another person’s information.

Website text, graphics, branding, and other materials belong to Clear Choice or their respective owners, except where otherwise identified. You may view or print reasonable portions for your own insurance-related use. Other uses require the relevant owner’s permission unless permitted by law or an applicable license. Carrier names and marks remain the property of their owners and do not imply that Clear Choice is the carrier.

9. Third-party websites and services

Links may take you to insurer portals, quote systems, payment services, or other independently operated websites. Their terms and privacy notices apply to their services. Review them before submitting information. A link does not guarantee another site’s accuracy, availability, or security, and these terms do not eliminate any responsibility we have under applicable law for our own conduct.

10. Website availability and responsibility

We make reasonable efforts to keep website information useful and current, but errors and interruptions can occur. To the extent permitted by law, the website is provided “as is” and “as available,” without warranties concerning uninterrupted access, error-free operation, or the suitability of general website information for your individual circumstances.

To the extent permitted by law, Clear Choice is not liable for indirect or consequential losses caused solely by website outages or technical errors. This provision does not limit liability for fraud, willful misconduct, gross negligence, a breach of duties owed in providing insurance services, or any liability that cannot lawfully be limited. It does not modify insurance coverage or an insurer’s obligations.

11. Changes and applicable law

We may update these terms by posting a revised version with a new effective date. Changes apply prospectively and do not alter existing policy contracts or accrued rights. Where the law requires additional notice or agreement, we will obtain it before relying on a change.

Arizona law governs these website terms, except where applicable federal law or mandatory protections under another jurisdiction’s law require otherwise. Nothing in these terms prevents you from making a complaint to an insurance regulator or exercising a nonwaivable legal right. If a provision is unenforceable, the remaining provisions continue to apply to the extent permitted by law.
