Clear Choice Insurance, LLC (“Clear Choice Insurance,” “we,” “us,” or “our”) is an Arizona independent insurance agency. This policy describes how we handle personal information collected through our website, inquiries, and related agency services. Our website does not offer customer accounts or accept online payments.
Insurance companies, consumer reporting agencies, and other independently operated services may have their own privacy notices. Additional insurance privacy notices and authorizations may apply to an application or transaction. We provide additional notices and obtain authorizations when required by law. This website policy is not a substitute for a required transaction-specific notice or authorization.
2. Information we collect
Information you provide. When you request contact through our website, we collect the information submitted, such as your name, email address, optional telephone number, coverage interest, and contact request or consent. If you communicate with our team, we may maintain correspondence and information needed to respond.
Information for insurance services. If you proceed with a quote, application, policy review, or service request, we may need additional information relevant to that transaction. Depending on the insurance involved, this may include your address, date of birth, household or driver information, vehicle or watercraft details, business operations, insurance history, and coverage needs. More sensitive information should be provided only through a method designated for that purpose.
Information from other sources. Where relevant and legally permitted, we may receive information from insurers, insurance support organizations, authorized representatives, public records, or consumer reporting agencies. Depending on the product, insurers or authorized service providers may obtain driving records, claims-history reports, or other underwriting information. We obtain authorizations and provide notices when required; simply visiting this website is not an authorization to obtain a consumer report.
Information supplied by an insurance support organization may be retained by that organization and disclosed to others as permitted by law and its own applicable notices.
Technical information. Website hosting, security, and related service providers may process information such as IP addresses, browser or device details, pages requested, timestamps, and error logs when delivering or protecting the website. The tracking technologies actually used on this website are described in Section 5.
Please do not submit Social Security numbers, driver’s license documents, payment details, medical records, or other sensitive information through a general inquiry form or ordinary email. Ask us for the appropriate method first.

A proposed business transfer does not create an unrestricted right to disclose insurance information.
Advertising and independent marketing. We do not use advertising cookies, and we do not sell or share inquiry information or leads with unaffiliated businesses for their own marketing. This does not prevent permitted disclosures to insurers and service providers for the insurance services you request, as described above.
We make disclosures requiring your authorization only after obtaining that authorization, and provide any required opportunity to limit marketing disclosures. A general website inquiry is not a blanket disclosure authorization.

A general inquiry does not by itself authorize marketing texts or automated marketing calls. Separate disclosures and consent are required where applicable. If you do not want to provide information through the website, you may contact the agency directly; some information may still be necessary to provide a quote or service.
7. Arizona insurance-information rights
Where Arizona’s insurance privacy provisions apply, you may submit a written request, with appropriate identification, for access to reasonably described personal information that we can locate and retrieve. Subject to statutory exceptions, we will respond within 30 business days. Applicable rights include seeing or obtaining a copy of recorded information, identifying institutional sources, and receiving information about certain disclosures during the preceding two years. A reasonable copying fee may apply where permitted.
You may also request that disputed recorded information be corrected, amended, or deleted. Where the law applies, we will act on the request or explain a refusal and your right to submit a statement of disagreement within 30 business days. If you submit a qualifying statement, we will associate it with the disputed record and include it with relevant disclosures as required. Corrected information and statements are communicated to appropriate recipients as required by law.
These rights have exceptions, including certain information collected in connection with claims or legal proceedings. They do not establish an unrestricted right to erase accurate insurance records or records that must be retained. Other applicable laws may provide additional rights.
To make a request, email or mail the privacy contact in Section 11. Describe the information and requested action. We may ask for information reasonably needed to verify your identity and authority, but do not send sensitive identity documents until we provide appropriate instructions.
8. Security and retention
We use reasonable administrative, technical, and physical safeguards appropriate to the information we handle. No website, transmission method, or storage system can be guaranteed completely secure.
We retain information for as long as reasonably necessary for the purposes described here, taking account of applicable insurance recordkeeping requirements, ongoing customer service, legal obligations, dispute resolution, and security needs. Different records may require different retention periods.
A contact form that sends an email can still result in information being retained in agency inboxes, mail-provider systems, server logs, backups, or agency records. The absence of a form-submission database does not mean that information is not retained.
9. Children and information about other people
Our website is intended for a general adult audience and is not directed to children under 13. We do not knowingly solicit personal information directly from children under 13 through its inquiry features. Contact us if you believe a child has submitted information inappropriately.
Information about household members, dependents, or other insured persons may be necessary for an insurance transaction and may be provided by an authorized adult or representative. That information is handled according to applicable law and the relevant insurance notices.
